Please note that this questionnaire will take about an hour of your time (or more). There are XXX pages of questions, and while you don't have to answer them all, I do indeed suggest attempting them all at the very least. PLEASE SKIP any questions that are irrelevant to you.

THIS IS MEANT TO BE A CHALLENGE. And you won't have answers to all questions. And that is totally a-ok. If you don't understand a question, leave it blank or let me know what you don't understand. (Some of the questions are there to illuminate hidden spots you might not have awareness of).
Once you've completed the questionnaire, you'll be directed to set up our call. (You'll schedule 45 minutes with me. It will be 20-25 minutes of me downloading what I see, and then 10-15 minutes of Q+A. And you'll get a recording when we're done!)
Now... onto the form itself! How does one approach this? Well…All the questions are optional, but... the more information you share with me, the better I can support you. And even the ‘hard’ questions are there on purpose. This isn’t a one-size-fits-all approach! Applicants will be assigned to the level of coaching that best suits their needs based on the information below. So...let’s dive in shall we?
